DEPARTMENT RECOMMENDATION: 

Health and Human Services Agency (HHSA) recommending the Board approve Agreement 161-F1811 with County of Plumas to serve as the Host Entity to collect and disburse participation fees to County of El Dorado as a Local Government Agency  (LGA) providing Medi-Cal Administrative Activities and Targeted Case Management (MAA/TCM). This funding Agreement provides federal reimbursement for qualifying MAA/TCM services. Additionally, County of Plumas requires a participation fee. The MAA/TCM participation fee is determined annually based on the percentage of revenue received by each program. The fiscal year (FY) 2016-17 participation fee for the County of El Dorado was $9,672.

 

 

DISCUSSION / BACKGROUND:

The California Department of Health Care Services (DHCS) is responsible for administering the California Medi-Cal Assistance Program (CMAA), of which MAA and TCM are components.  The State enters into cooperative arrangements with LGAs, utilizing a Host Entity model, to ensure the appropriate administration of MAA and TCM with the objective to assist eligible Medi-Cal recipients in gaining access to needed medical, social, educational, and other services. The LGA (County of El Dorado) may claim federal Medicaid matching funds for assisting the State in the proper and efficient administration of the MediCal Program.  The Health and Human Services Agency acts as the liaison for receipt of MAA and TCM revenues for the County and has been participating in MAA and TCM since 1998.

 

DHCS contracts with one "host entity" to manage and collect the LGA's share of actual costs to administer the CMAA program.  The Host Entity (in this case County of Plumas) contracts with DHCS.  Once the "host entity" agreement between the State and County of Plumas is fully executed, the County of Plumas then sends contracts to the participating counties.   Upon receipt of Agreement 161-F1811 from the County of Plumas (July 17, 2017), HHSA immediately processed the agreement for the next available Board Agenda.

 

Pursuant to Welfare and Institutions Code §14132.47(m), LGAs participating in MAA and TCM are required to pay an annual participation fee through a mechanism agreed to by the State and the LGA Consortium.  The current agreed-to mechanism assigns administrative and fiscal intermediary duties between the State and all participating LGAs to a host entity county.  The County of Marin was previously designated as the MAA/TCM host entity through June 30, 2014.  Beginning July 1, 2014, the County of Plumas became the designated MAA/TCM host entity.

 

HHSA Public Guardian participates in MAA/TCM to claim revenue for the program.

 

The current Agreement (250-M1510) with Plumas County expired June 30, 2017 and was approved by the Board on January 6, 2015 (File ID 14-1657).

 

ALTERNATIVES:

The Board could choose to not participate in this Agreement, thereby potentially eliminating the MAA/TCM program, as Agreement 161-F1811 is required for the County’s continued participation in MAA and TCM. If the Agreement is not approved, loss in revenue could reach an estimated $600,000 in funding for programs participating in MAA and TCM, which could have a significant impact on General Fund.

It is recommended that the Board approve this item.

 

FINANCIAL IMPACT:

There is no Net County Cost associated with approval of this Agenda item.  Sufficient appropriations are included in the Fiscal Year 2017-18 Budget.

1)  Clerk of the Board to obtain the signature of the Chair on three (3) originals of Agreement 161-F1811.

2)  Clerk of the Board to return all documents to HHSA Contracts Unit at 3057 Briw Road, Suite B, to obtain final signature from Plumas County.
