Fiscal Impact/Change to Net County Cost:  The Agreement is funded by State SB163 Wraparound Program Allocation savings with required County match met primarily with realignment resources.  Funds are budgeted for FY 2008-09 and will be budgeted for FY 2009-10, FY 2010-11 and FY 2011-12.
 
Background:  The El Dorado County SB 163 Wraparound Program was established in 2001.  SB163 funds services to youth at risk of out-of-home placement or at risk of being placed in higher levels of care with the goal of reducing the number of youth placed in group homes.  On February 28, 2006, the Board of Supervisors approved a proposal to transfer administrative responsibility of the SB163 Wraparound Program to the Department of Human Services and allow for expansion of services.  On September 26, 2006, the Board approved a revised El Dorado County Wraparound Program Plan ("Plan"), which was approved by the State on December 18, 2006.  The Plan prioritized and authorized the expenditure of SB 163 Wraparound Program fund savings realized beyond necessary expenditures for the authorized six slotted youth, for, among other services, emergency interventions or other child welfare services that benefit at-risk youth and their families in our community as authorized by the County's Interagency Advisory Council (IAC). In 2008 the Plan was revised again to reflect minor changes that clarify operational roles and responsibilities of the Department of Human Services and the Mental Health Department.  The revision was approved by the IAC on April 9, 2008 and approved by the Board on August 26, 2008.  The State approved the revised Plan on October 9, 2008.
 
Procurement and Contracts executed a prior Agreement #201-S0711 with Maxim Healthcare Services, Inc. on December 1, 2006 for a two-year term ending November 30, 2008 in the amount of $40,000 for the provision of supplemental licensed health care provider services on an "as requested" basis for at-risk youth who were clients of the Department of Human Services.  The Board approved Amendment I to Agreement #201-S0711 on January 29, 2008 to increase the compensation by $30,000 for a total not-to-exceed amount of $70,000.  
 
Reason for Recommendation: The provision of supplemental licensed health care provider services on an as-requested basis for at-risk youth continues to prove to be a successful tool to improve the lives of families served by the Department of Human Services.  Maxim Healthcare Services, Inc. provides 24-hour emergency staffing for one-to-one assistance with children who are clients of Child Protective Services and the SB 163 Wraparound Program.  Agreement for Services #344-S0911 with Maxim Healthcare Services covers a three-year term, commencing December 1, 2008 through November 30, 2011 with a not-to-exceed compensation amount of $75,000.  
 
The prior Agreement for Services with Maxim Healthcare Services, Inc. expired on November 30, 2008.  The new Agreement is being submitted for retroactive execution due to a delay in obtaining vendor signatures and to prevent an interruption in the availability of this critical service.  
 
Action to be taken following Board approval:
1.  Chairman to sign two (2) original Agreements
2.  Board Clerk to return one (1) original Agreement to Department.  
3.  Department to distribute as appropriate.
