Fiscal Impact:  The cost of this amendment is $25,000 for a total contract amount of $77,500.  Funding for this contract is included in the Department's budget and in the approved MHSA Community Services and Supports (CSS) Plan.  There is no Change to Net County Cost.
 
Background:  One of the programs identified in the County's Community Services and Supports (CSS) plan under MHSA provides outreach and engagement for unserved and underserved target populations, such as Native Americans.  More specifically, the plan calls for the Mental Health Division of the Health Services Department (MHD) to pursue collaboration with the local Native American community for case management, and data collection to develop a better understanding of the specific mental health challenges affecting Native Americans.
 
The vendor is providing case management services through one (1) full-time employee (FTE) and is expected to maintain an average of 25 open cases and work collaboratively with the County to provide mental health services to Native Americans who have serious mental illness.  Case management will assist these individuals with independence, self sufficiency, wellness and recovery, in the context of their culture.  Examples of case management services include:  vocational assistance; teaching independent life skills; referrals; and accompanying clients to medical care appointments to encourage adoption of appropriate health promotion/disease prevention skills.
 
Additionally, the vendor assists the MHD to assess local Native Americans mental health needs by producing a report describing the mental health status of County Native Americans, accompanied by recommendations to address the issues identified and a strategic plan for carrying out those recommendations.  This work involves accessing existing data sets from the Native American Resource Collaborative; accessing and collecting data on the target population directly and through other tribal organizations; producing recommendations to address the issues identified; and producing a strategic plan to implement the proposed recommendations.
 
Reason for Recommendation:  On August 26, 2008, the Board approved this contract for an eight month period ending December 31, 2008. This amendment extends the contract to June 30, 2009, consistent with the current approved MHSA CSS plan and is not affected by the proposed MHSA CSS plan update. The value of the contract will be increased with this amendment to cover the additional six month period of services to be provided.
 
 
Action to be taken following Board approval:   
 
1.      Chairman will sign two (2) original Amendments.
2.      Board Clerk's Office to return one (1) original Amendment to the Health Services Department, Mental Health Division.
3.      Mental Health Division to distribute as appropriate.
 
Concurrences:  The original agreement was approved by County Counsel and Risk Management and the amendment is currently under review.
