Fiscal Impact:  The cost of this amendment is $49,720 for a total contract amount of $99,440.  Funding for this contract is included in the Department's budget and in the approved MHSA Community Services and Supports (CSS) Plan.  There is no Change to Net County Cost.
 
Background:  One of the programs identified in the County's Community Services and Supports (CSS) plan under MHSA is a transitional housing program for adults. The Department has contracted with Choices Transitional Services since November, 2007 to provide transitional housing services on an "as requested" basis.
 
This amendment will allow the Department to continue providing assistance in the areas of transitional housing and independent living skills for adults. Services provided under this contract include: transitional housing, case management, house management and supervision services. The vendor maintains a bed capacity for placement of up to six clients referred by the Department. The clients referred have serious mental health issues, and are either homeless or at risk of homelessness.
 
The goal of this program is to encourage independence and self sufficiency in referred clients. The vendor provides skills training for clients including but not limited to: budget planning, shopping, cooking, use of available transportation systems, and general mobility in the community.
 
Reason for Recommendation:  On August 26, 2008, the Board approved this contract for a six month period ending December 31, 2008. This amendment extends the contract to June 30, 2009, consistent with the current approved MHSA CSS plan and is not affected by the proposed MHSA CSS plan update. The value of the contract will be increased with this amendment to cover the additional six month period of services to be provided.
 
Action to be taken following Board approval:   
 
1.      Chairman will sign two (2) original Amendments.
2.      Board Clerk's Office to return one (1) original Amendment to the Health Services Department, Mental Health Division.
3.      Mental Health Division to distribute as appropriate.
 
Concurrences: The original agreement was approved by County Counsel and Risk Management and the amendment is currently under review.
