DEPARTMENT RECOMMENDATION: 

The Health and Human Services Agency is recommending the Board receive and file a presentation on the status of Service Integration Strategic Plan Goal #5.

 

DISCUSSION / BACKGROUND:

The Health and Human Services Agency (HHSA) has been pursuing their Strategic Plan to determine the future direction of the Agency, looking three to five years ahead and charting a course based on strong indicators of the HHSA environment in future years.

 

Services Integration is goal number 5 in the HHSA strategic plan.  HHSA is working to improve the way we coordinate and provide services.  The goal is to develop and implement a continuum of care model for individuals, families and community partners that integrates access, assessment, referral and efficient delivery of programs and services.

 

HHSA has many programs and services that benefit our communities.  However, programs often work independently of each other due to restricted funding sources, with separate physical locations, duplicative forms, and limited coordination of complimentary services.  Often clients interact with multiple case workers depending on their need which can be confusing and add frustration to the client.  Service Integration can help HHSA streamline processes, maximize limited resources and work together within our complex service delivery systems.

 

This presentation will outline the objectives and tasks that have been identified and executed in the pursuit of the service integration goal. HHSA will continue to provide the same programs, but how those programs are organized with other programs, and how they work together will change with the ultimate goal of improved client outcomes.
