Fiscal Impact/Change to Net County Cost: None.

Background: El Dorado County Health Services Department, on behalf of ACCEL (Access El Dorado - a coalition of safety net providers) has been awarded funds to assess specialty care shortages and develop an implementation plan for innovative delivery models to improve access to healthcare for County residents. The goals of this 21-month implementation proposal are to: 1) Establish project infrastructure and participant commitments; 2) Develop a standard template for Specialty Care Consult Pathways; 3) Implement Specialty Care Consult Pathways adapted for Orthopedics (2009) and Pain Management (2010) that incorporate practice guidelines and continuing medical education (CME) for primary care providers; and 4) Assess the effectiveness of specialty care consult pathway, guidelines and primary care expanded scope of practice.

The funder will provide scheduled payments based on completion of grant deliverables in the amount of $100,000 in FY 2008-09; $75,000 in FY 2009-10; and the remaining $75,000 in FY 2010-11.
