Fiscal Impact/Change to Net County Cost: MHD will receive $32,937 in funding for FY 09-10 to provide for the transition of homeless/resource-less persons leaving the Psychiatric Health Facility (PHF) or the Crisis Residential Treatment (CRT) facility to appropriate placement in the community. The Mental Health Division uses State Mental Health Services Act (MHSA) and Realignment funds to satisfy the required 1:3 match of $10,979. There is no Net County Cost associated with this grant.

Reason for Recommendation: Since the early 1990’s, the Mental Health Division has applied for and received a PATH grant each fiscal year to provide for the responsible and efficient transition of homeless and resource-less persons from the PHF into appropriate placement in the community. The application for the FY 09-10 grant will also address the transition of this population from the new Crisis Residential Treatment (CRT) facility into the community. The grant funds portions of the salaries and benefits of one discharge planner, two case managers, and two rehabilitation workers that work to transition these clients into appropriate housing when they leave the PHF or CRT.
