Health and Human Services Agency (HHSA) recommending the Board:
1) Approve and authorize the Chair to execute Agreement for Services 4206 with Progress House, Inc., for the provision of Drug Medi-Cal Organized Delivery System Services, effective upon execution through December 31, 2020, with a maximum obligation of $1,268,247 for the term of the Agreement;
2) Find that in accordance with County Ordinance 3.13.030 it is feasible to contract with Progress House, Inc., for services provided under this Agreement 4206 because (B) “Specialty skills and qualifications not expressly identified in classifications are involved in the performance of the work;” and
3) Approve and authorize the Director of HHSA to execute further documents relating to Agreement for Services 4206, including amendments which do not increase the maximum dollar amount or term of the Agreement, and contingent upon approval by County Counsel and Risk Management.
FUNDING: Federal Medi-Cal with matched funding from 2011 Behavioral Health Realignment and State General Fund for Residential and Outpatient Intensive Treatment services for the expanded population.

DISCUSSION / BACKGROUND:
On May 23, 2017, the Board authorized the County to “opt in” to the Federal 1115 Waiver for a Drug Medi-Cal Organized Delivery System (DMC-ODS) (File ID: 17-0525).
Since that time the Health and Human Services Agency has developed service agreements with providers to provide substance use disorder (SUD) services for County clients in accordance with the terms and conditions of the DMC-ODS agreement by and between the State and County of El Dorado.
During this time HHSA and Progress House have continued to operate with the original SUD agreement #8(002-S1811) (File ID: 17-0289) while working through some issues pertaining to prior year(s) services. At this time having successfully navigated those issues, and to ensure compliance with the DMC-ODS system of care, HHSA is now recommending the Board approve Agreement #4206 for services under the DMC-ODS system.
ALTERNATIVES:
Should the Board decline to approve this recommendation, HHSA would continue to provide services through existing contracts; however, it could result in difficulty meeting the needs of the community, and could result in HHSA being out-of-compliance with the DMC-ODS Waiver Plan.

It is recommended that the Board approve this item.
FINANCIAL IMPACT:
Services provided under the DMC-ODS program will be funded by Medicaid Federal Financial Participation (FFP) funds that will be matched with 2011 Behavioral Health Realignment funds. Since many of the individuals in need of these services are Medi-Cal beneficiaries, it is estimated that 90-95% of the services provided will be covered by Drug Medi-Cal and the remainder would be paid out of Behavioral Health Realignment. The County has negotiated rates with the State Department of Health Care Services.
2011 Realignment funds would provide the match from the Behavioral Health Subaccount. It should be noted that 2011 Realignment is funded by a dedicated portion of statewide sales tax, and as long as statewide sales tax continues to grow, “Growth” dollars are available to be allocated to counties. Growth funds are allocated by formula to counties, 50% weighted by approved County claim amounts, and 50% weighted toward the County’s share of Statewide Medi-Cal beneficiaries. Should 2011 Realignment funds decline, HHSA would first identify appropriate backup funding (e.g. MHSA Full Service Partnership funding for qualified clients) before requesting General Fund.
