Fiscal Impact/Change to Net County Cost: These expenditures are budgeted within the Risk Management internal service fund, health benefits sub-fund. Revenue is provided by Board-approved internal charges, retirees and outside agencies on behalf of participants in the County's
fully-insured PacifiCare program. This contract is for a one year term with an annual estimated cost of $23,000.

Reason for Recommendation: This agreement is the direct result of UHC PacifiCare's decision to replace the former embedded Vision Plan through VSP to a vision plan within the UnitedHealthcare Specialty Benefits program. The UHC Vision Plan services will include both claims administration and a network of providers for participants of the County's fully-insured PacifiCare program.

The County's self-funded vision plan with Vision Service Plan (VSP) remains in place for participants in the County's Blue Shield self-insured program.

It is to be noted that as part of the terms of the contract, UHC reserves the right to change the schedule of rates for Premiums, after a 60 day prior written notice on any premium due date and that the Policy automatically terminates if payment of premium is not made within 31 days of last day for which premiums were paid.
