FUNDING:   California Department of Aging Dignity at Home Fall Prevention Program grant.

DISCUSSION / BACKGROUND:

On March 17, 2020 with Legistar item 20-0198, the Board accepted and authorized the Chair to sign Dignity at Home Fall Prevention Program Grant Agreement FP-1920-29 in the amount of $143,750 for the purpose of providing fall and injury prevention information, education, referral services, equipment, assessments, services, materials and labor costs to the eligible service population. 

 

Fall Prevention Program funding is intended to target services and activities specific to individuals who meet the following criteria:  (1) Age sixty (60) or have disabilities, (2) whose adjusted household income does not exceed 80 percent (80%) of the area median income, and (3) who are at risk of falling or institutionalization.

 

On February 23, 2021 HHSA received the Amendment from CDA and it must be returned by June 1, 2021.

 

ALTERNATIVES:

The Board could choose to not accept the term extension for the funding, thereby shortening the potential usage of this Fall Prevention Program funding from the Senior Services programs funding opportunities.

CAO RECOMMENDATION:

Approve as recommended.

 

FINANCIAL IMPACT:

There is no additional funding provided in the Amendment.  With the extended term, the funds will be budgeted in FY 2021-22 Budget to offset the cost of fall prevention services with no impact to the General Fund.

1) Clerk of the Board to obtain Chair signature in blue ink on four (4) originals of Amendment 1 to Agreement FP-1920-29; and

2) Return four (4) executed originals to HHSA Contracts Unit at 3057 Briw Road for further processing; and

3) Return one (1) Certified Minute Order to HHSA Contracts Unit at 3057 Briw Road.
