Title
Human Services Department recommending the Board authorize the Chairman to sign a grant application to be submitted to Marshall Foundation for Community Health, requesting $5,000 in grant funding for support services to vulnerable elderly residents and their caregivers at the County's Senior Day Care center.

FUNDING:  Marshall Foundation for Community Health Grant.
Body

Fiscal Impact/Change to Net County Cost:  No change.
There is no County share of cost or match requirement associated with the Marshall Foundation Grant.  If the grant is awarded, a Budget Transfer Request will be submitted to incorporate the new revenues and corresponding appropriations.

Reason for Recommendation:
The Department of Human Services (DHS) received notice of this funding opportunity from the Marshall Medical Foundation for Community Health on October 6, 2009.  Development of the application was expedited to ensure submittal by the November 9, 2009 deadline.  If awarded, the funds will be used to help offset the loss of approximately $75,000 in State Alzheimer's Day Care Resource Center funding, which occurred effective September 30, 2009.  This grant funding will assist the Senior Day Care Center in continuing to provide support services to adults who are frail, socially isolated, or functionally or cognitively impaired and their caregivers.  Services include respite, care plan development and management, specialized dementia care, assistance with medication and personal care, and coordination of daily activities.  Letters of support for the grant application have been requested from the Human Services Family Caregivers Support Program, the Del Oro Caregiver Resource Center and Partners in Care, and will be attached to the application at time of submittal.

Action to be taken following Board approval:
1.      Board Clerk to obtain the Chairman's signature on three (3) copies of the application and call DHS staff at x4836 or x7312 for pick up.
2.      DHS to pick up two (2) signed applications for submittal to Marshall Foundation for Community Health by the November 9, 2009 deadline.
