Fiscal Impact/Change to Net County Cost:  The total funding request has been anticipated in the Health Services Department, Mental Health Division, FY 09/10 budget. There is no Net County Cost associated with these MHSA items.
 
Background:  In November 2004, California voters passed Proposition 63, now known as the Mental Health Services Act (MHSA). The Act addresses multiple components and a broad continuum of mental health services and supports for residents of California. The Prevention and Early Intervention (PEI) component provides support at the earliest possible signs of mental health problems, addresses disparities in access to mental health services, and builds capacity for providing intervention services at sites within the community.
 
Reason for Recommendation:
 
Prevention and Early Intervention (PEI) Plan - $635,997
The Department is recommending the submittal of a PEI Plan request for $635,997 to be used for the programs identified below using existing positions where mentioned:
 
1.      School-Based Mental Health Promotion & Service Linkage      $142,083
Funding will be used to support County Mental Health clinical staff to participate in a school-based screening team providing early identification, assessment, and referral to MHSA-funded services. The target population is youth struggling with school success assessed to be related to family stress.  The request includes funding for 1.0 FTE Mental Health Clinician and 0.1 FTE Mental Health Program Coordinator.
 
2.      Primary Intervention Project (PIP)            $86,851
This early intervention strategy will be available on a pilot basis in two regions of the County serving youth in grades K-3.  The request will fund 0.65 FTE Mental Health Aides and 0.05 FTE Mental Health Program Coordinator within the Mental Health Division. The County Office of Education will provide appropriate space, as well as an MHSA funded, school-employed mental health professional and a coordinator to manage data collection and reporting. The continued use of this model, and potential future pilot sites, will be the subject of further discussion.
 
3.      Incredible Years      $21,360
This request will fund Mental Health Clinicians (0.07 FTE) and Mental Health Workers (0.16 FTE)  to conduct 12-14 week developmentally-based educational programs targeting 2-12 year old children, their parents and school teachers.  The goal is to offer this program at community-based settings, three times per year, to address needs in outer-lying communities.   
 
4.      Community Education Project            $14,000
This program funds three distinct education and outreach programs, primarily to serve El Dorado County youth and families. The programs emphasize the key role of family and promote mental health through knowledge, education, and skills building.  Funding for the programs will offset costs for materials, training, and related expenditures.
 
5.      Wennem Wadati - A Native Path to Healing            $51,216
This request funds a locally designed and culturally-specific intervention strategy to address gaps in service, health disparities and the effects of traumatic stress in the local American Indian community.
 
6.      Home Delivered Meals Wellness Outreach Program            $61,686
This project will enhance the existing Home-Delivered Meals program provided by the County Human Services Department and will provide outreach, screening, and early intervention for older adults who may be isolated, depressed, and/or suicidal. The request includes funding for 0.12 FTE Psychiatric Technician; 0.12 Nurse Practitioner/Physician Assistant, 0.25 Mental Health Worker, and 0.05 Mental Health Program Coordinator.  
 
7.      Health Disparities Initiative            $153,841
This Initiative was initially designed, approved, and implemented as part of the County's MHSA Community Services and Support (CSS) Plan. Experience with the program in recent years has demonstrated the effectiveness of brief and early prevention/intervention strategies; accordingly, this Initiative is now proposed to be transferred from CSS to the PEI component. This shift will allow the emphasis, goals, and evaluation process to focus on the influence of risk factors and prevention, as opposed to the alleviation of symptoms or the elimination of functional impairment as measured by Medi-Cal medical necessity guidelines. Services will continue to be provided by community-based agencies, with a 0.05 FTE allotment for a County liaison/Utilization Review Coordinator.
 
8.      Administration                  $104,960
The Department is requesting administrative funds equal to 16.5% of the PEI expenditures.
 
The Mental Health Division anticipates that future MHSA allocations will be sufficient to support these programs.  
 
Training, Technical Assistance and Capacity Building Request - $43,400
The Division is recommending submittal of a request for PEI Training, Technical Assistance and Capacity Building Project funds in the amount of $43,400 to better mobilize community strengths, as well as to gain further understanding and implementation of a prevention approach, reduce stigma and discrimination, and improve cultural competence.  
 
MHSA CSS and PEI FY09/10 Prudent Reserve - $397,329
In addition, the Mental Health Division is recommending submittal of the MHSA CSS and PEI FY 2009/10 Prudent Reserve Funding Request.  As with the previously established Prudent Reserve policy, the County must maintain a Prudent Reserve equal to 50% of the most recent annual approved CSS and PEI funding levels.
State Department of Mental Health Information Notice 09-16 allows counties to use PEI funds made available prior to FY 2008/09 to fund their Prudent Reserve.  Of the FY 2007/08 PEI allocation of $431,100, El Dorado County previously requested $89,700 for PEI planning. These funds were approved and received; however, of the $89,700, $55,929 is in our County Treasury and remains unspent (representing available carryover funding).  The remaining FY 2007/08 allocation of $341,400 has not yet been requested or received. Therefore, a total of $397,329 is requested to be added to the Prudent Reserve to include all unexpended funds from the FY 2007/08 allocation, as follows:

PEI funds have a three-year reversion date.  Therefore, the FY 2007/08 funds must be spent or placed in the Prudent Reserve by June 30, 2010 or they will revert back to the State.  By adding these funds to the Prudent Reserve, the Division will appropriately fund this required Reserve and will avoid any potential reversion of the FY 2007/08 funds.
 
Summary
The attached documents reflect input and feedback from the community collaborative planning process.  The PEI documents have each been posted for the required 30-day community review process, and Public Hearings were held at two locations:  Mental Health Clinic, 670 Placerville Dr, Placerville & Mental Health Clinic, 1900 Lake Tahoe Blvd, South Lake Tahoe.  While the Department has received and incorporated community feedback effecting program content and expenditures, some additional minor, corrections/edits may still need to be incorporated into the attached documents prior to sending the documents to the State.  
 
Action to be taken following Board approval:  The Health Services Director will sign and submit to the State the finalized MHSA PEI documents, including:
1.      Prevention and Early Intervention (PEI) Plan;
2.      Training, Technical Assistance and Capacity Building Funds Request Form (PEI Statewide Project); and the
3.      MHSA Community Services and Supports (CSS) and Prevention and Early Intervention (PEI) FY 2009/10 Prudent Reserve Funding Request.
 
Staff will return to the Board with any resultant contract documents that may require Board approval and/or signature.
