Title: Health and Human Services Agency (HHSA) recommending the Board: 1) Make findings in accordance with County Ordinance 3.13.030(C) that it is appropriate to contract with El Dorado County Community Health Center for the provision of Public Health services, provided under Funding Agreement 6498, because pursuant to County Ordinance 3.13.030(C) “the limited timeframes, temporary or occasional nature, or schedule for the project or scope of work, the ongoing aggregate of work to be performed is not sufficient to warrant addition of permanent staff;” 2) Approve and authorize the Chair to sign Funding Agreement 6498 with El Dorado County Community Health Center for the purposes of preventing and controlling vaccine-preventable diseases within the County, with a not-to-exceed (NTE) amount of $450,000, and for the term of upon execution through June 30, 2022 with the option to extend for up to two (2) one-year terms with no increase to the NTE; and 3) Authorize the HHSA Director, or designee, t...

Health and Human Services Agency (HHSA) recommending the Board:
1) Make findings in accordance with County Ordinance 3.13.030(C) that it is appropriate to contract with El Dorado County Community Health Center for the provision of Public Health services, provided under Funding Agreement 6498, because pursuant to County Ordinance 3.13.030(C) “the limited timeframes, temporary or occasional nature, or schedule for the project or scope of work, the ongoing aggregate of work to be performed is not sufficient to warrant addition of permanent staff;”
2) Approve and authorize the Chair to sign Funding Agreement 6498 with El Dorado County Community Health Center for the purposes of preventing and controlling vaccine-preventable diseases within the County, with a not-to-exceed (NTE) amount of $450,000, and for the term of upon execution through June 30, 2022 with the option to extend for up to two (2) one-year terms with no increase to the NTE; and
3) Authorize the HHSA Director, or designee, to execute further documents relating to Funding Agreement 6498, including amendments that do not increase the maximum dollar amount or term of the Agreement beyond June 30, 2024, contingent upon approval by County Counsel and Risk Management.

FUNDING: 100% California Department of Public Health Immunization Funding.

DISCUSSION / BACKGROUND:
Health and Human Services Agency (HHSA,) as the primary recipient of the California Department of Public Health (CDPH) Immunization Grant Agreement 17-10315 (Grant), has identified service needs that fall within the scope and purpose of the grant, and has submitted a budget and work plan to CDPH to sub award funds to a community partner for the purposes of preventing and controlling vaccine-preventable diseases within the County. El Dorado County Community Health Center (EDCCHC) is a community partner identified in the CDPH budget and work plan, currently providing community services within El Dorado County.
