Fiscal Impact/Change to Net County Cost:  
No impact to Net County Cost.  The required 20% local cash or in-kind match for the grant will be provided by the El Dorado County Community Health Center.
 
Background:
The County Medical Services Program (CMSP) is authorized by Code to provide for specified health services to eligible County residents.  Counties are responsible for provision of basic health care to medically indigent residents.   The El Dorado County Health Services Department receives funding for and administers the CMSP under provisions of an MOU with the Statewide CMSP Governing Board.  Human Services determines eligibility, issues benefits and provides case management for CMSP clients.  
 
Reason for Recommendation:
The El Dorado County Community Health Center (EDCCHC) is applying for a County Medical Services Program (CMSP) Pilot Project planning grant to develop a plan to create a healthcare "pathway" for the El Dorado County's homeless to encourage and enable them to access basic health care through CMSP, using volunteer "ambassadors" to help guide the homeless to appropriate primary and preventative health care, thus reducing the incidence of Emergency Room visits and Emergency Medical Services rides.  The CMSP Pilot Project Request for Proposals (RFP) allows for targeting homeless adults and requires demonstrated capacity to bring local stakeholders together and the desire to develop an implementation plan that addresses all requirements for future implementation as detailed in the letters and in the attached RFP Summary.  
 
Letters of commitment/support from Human Services and Health Services Departments are a required component of the application.  Future CMSP Pilot Project funding rounds will provide the opportunity for EDCCHC to apply for up to $150,000 per project year, for up to three years, to implement the plan developed as a result of this planning grant.  
 
The Human Services and Health Services Directors are committed to participating (or assigning a designee to participate) in several meetings of the planning team led by EDCCHC to assist in their effort to develop a viable plan for linking the targeted homeless adult population group with appropriate health services.  No additional commitment of County staff or financial resources is required as a result of providing these CMSP Grant letters of commitment to EDCCHC.  
 
Action to be taken following Board approval:
Health Services and Human Service to provide the El Dorado County Community Health Center with letters of commitment for submittal with their CMSP Pilot Project Grant Application by the March 5, 2010 deadline.
