Fiscal Impact/Change to Net County Cost:  The amount of this Amendment is $6,600 for a total contract amount of $114,000.  There is no Net County Cost associated with this Agreement.

 

Background:  One of the programs identified in the County’s approved MHSA funded Prevention and Early Intervention (PEI) Plan is a Health Disparities Initiative, addressing health disparities and culturally specific services for the Latino communities. The Board approved PEI Plan specifically calls for Family Connections to provide outreach and engagement services through the Promotora model.

 

The goal of the Family Connections program is to reduce barriers to mental health care access, thereby decreasing the health disparities experienced by the Latino population on the Western Slope. This goal is accomplished by use of cultural and linguistic peer outreach to engage the Latino community into critically needed services. The vendor staff funded by this program work collaboratively with County staff under the general supervision of the Mental Health Division Deputy Director or designee.

 

There is no change in the term of this Agreement, which is July 1, 2010 through June 30, 2011.

 

Compensation for Services:  Payments are made to Contractor based upon monthly invoicing at specified billing rates for actual services performed and documented per contract requirements. Services and rates are detailed in Exhibit C of the contract.

 

Contract Termination:  This Agreement includes the County’s standard Fiscal Considerations termination language, as well as a provision for the County to terminate the Agreement without cause upon seven (7) calendar days written notice.

 

Reason for Recommendation:  This Amendment will allow the contractor to add the position of Group Educator, and will add $6,600 to fund that position. The Group Educator will:

                     conduct ongoing weekly bilingual/bicultural groups to provide education and support for Latino adults and children;

                     conduct meetings to address health issues, life skills, social support needs, parenting and other topics; and

                     address mental health prevention issues, isolation, poverty, housing, violence prevention, and alcohol and drug abuse prevention.

 

Action to be taken following Board approval: 

1.                     Chair to sign three (3) original Amendments.

2.                     Board Clerk’s Office to return two (2) original Amendments to the Health Services Department, Mental Health Division.

3.                     Mental Health Division to distribute as appropriate.
