Fiscal Impact/Change to Net County Cost:  There is no Net County Cost associated with this Amendment I.  Amendment I does not modify the Not-To-Exceed amount of the original Agreement.
 
Compensation for Services:   Payments are made to Contractor based upon monthly invoicing at specified billing rates for actual services performed and documented per contract requirements. Services and rates are detailed on pages 2 and 3 of the contract.
 
Contract Termination:  This Agreement includes the County's standard Fiscal Considerations termination language, as well as a provision for the County to terminate the Agreement without cause upon seven (7) calendar days written notice.
 
Background:  The original Agreement 023-S1111 was approved by the Board of Supervisors on June 29, 2010 via Board item 10-0664.  Amendment I extends the term for three months, through September 30, 2011, to allow the vendor additional time to provide services using remaining MHSA funds allotted to this Agreement.
 
Reason for Recommendation:  One of the programs identified in the County's approved MHSA Community Services and Support (CSS) Plan is an employment assistance program for severely mentally ill adults, the goal of which is to enhance self-sufficiency and promote self-reliance in participants through finding and maintaining employment. The population that suffers from mental illness is among the most challenging groups of disabled individuals in relationship to the employment process.  Therefore, specialized services such as those provided by this contractor are critical to the success of this effort.
 
Through the proposed Amendment I, Crossroads will continue to provide vocational rehabilitation services and related skills training to program participants. The contractor uses an evidence based model designed specifically for this population called "supported employment."  Crossroads provides the following services to assist participants in finding and keeping employment: vocational assessment, pre-employment classes, individual support services in finding employment, employment preparation classes, job development and placement services, post-employment support, small business development, and additional support services as needed.
 
The vendor staff funded by this program work collaboratively with County staff under the general supervision of the MHD Deputy Director or designee.
 
Action to be taken following Board approval:  
 
1.      Chair to sign three (3) original Amendments.
2.      Board Clerk's Office to return two (2) original Amendments to the Health Services Department, Mental Health Division.
3.      Mental Health Division to distribute as appropriate.
