Fiscal Impact/Change to Net County Cost: The amount of this Amendment is $31,500 for a total contract amount of $90,000. There is no Net County Cost associated with this Amendment II.

Compensation for Services: Payments are made to Contractor based upon monthly invoicing at specified billing rates for actual services performed and documented per contract requirements. Services and rates are detailed on page 5 of the contract.

Contract Termination: This Agreement includes the County’s standard Fiscal Considerations termination language, as well as a provision for the County to terminate the Agreement without cause upon seven (7) calendar days written notice.

Background: One of the programs identified in the County’s Board approved MHSA funded Prevention and Early Intervention (PEI) Plan is a Wellness Outreach Program, addressing vulnerable adults who have risk factors associated with suicide and/or suffer from mental distress, depression and/or exposure to trauma and isolation. The PEI Plan recognizes the need to provide outreach, engagement, screening, assessment, early intervention mental health services, and linkage to appropriate community based organizations for vulnerable adults who may have limited support systems.
