Health Services Department, Mental Health Division, recommending the Board authorize the Chair to sign Amendment I to Agreement 023-S1111 with Crossroads Diversified Services, Inc., extending the term for three months through September 30, 2011, with no change in the not-to-exceed amount, to provide work readiness activities and employment assistance for mentally disordered adults on an “as requested” basis for said Department.
FUNDING: State Mental Health Services Act (MHSA) Community Services and Support component.

Fiscal Impact/Change to Net County Cost: There is no Net County Cost associated with this Amendment I. Amendment I does not modify the Not-To-Exceed amount of the original Agreement.
Compensation for Services: Payments are made to Contractor based upon monthly invoicing at specified billing rates for actual services performed and documented per contract requirements. Services and rates are detailed on pages 2 and 3 of the contract.
Contract Termination: This Agreement includes the County’s standard Fiscal Considerations termination language, as well as a provision for the County to terminate the Agreement without cause upon seven (7) calendar days written notice.
Background: The original Agreement 023-S1111 was approved by the Board of Supervisors on June 29, 2010 via Board item 10-0664. Amendment I extends the term for three months, through September 30, 2011, to allow the vendor additional time to provide services using remaining MHSA funds allotted to this Agreement.
